A physical exam is required upon entry into school, 6th grade and 11th grade.
A dental exam is required upon entry into school, Kindergarten, 3rd grade and 7th grade.
Prescription Medications that are to be administered during the school day will need an authorization for prescription medication form. The authorization form must be accompanied by physician’s instruction must include a description of the medication, and the proper dosage. The medication must be provided to the school in the original container, with a legible label. This release will be retained in the student’s medical file.
NON-PRESCRIPTION MEDICATIONS FORM
Non-prescription medication may be given for a specific, time-limited minor illness or for intermittent medical conditions. If the medication is needed for more than ten doses, a prescription may be necessary in order for the medication to continue to be given at school. The medication must be provided in its original, unopened container, with a legible label, and authorized for the dosage recommended for children on the package.
Students that will be absent for more than 1 school day should complete the Family Trip Form (preferably 10 days in advance).
Students that will be absent for a college visit should complete this form 3 days prior to absence.
